Teaching central line placement: no clear window

چکیده
We appreciate Maizel and colleagues addressing the teaching of ultrasound-guided central line placement to medical residents [1]; however, we have several comments. We feel that the success rates reported in this study are artificially high since most residents in 2014 are training with the use of ultrasound, and it is highly unlikely that the initial procedures being done by these senior residents (fourth and fifth year medical residents) were their first experience. In addition, by teaching the landmark technique second, the residents at that time are definitely experienced with the procedure of central line placement. The shorter procedure time noted for the landmark technique is likely related to the fact the practitioners are

Teaching Ultrasound Imaging for Central Line Placement—A Resident’s Perspective
The availability of ultrasound (US) devices has refined techniques for vascular access. The resident trainee’s ability to learn US techniques depends upon device availability, skills, and motivation of instructors. We hypothesized that hands-on teaching program of US techniques for vascular access would have a positive impact on the trainee’s perceptions. After IRB approval, PGY3 anesthesiology...
متن کاملReduction of central venous catheter associated blood stream infections following implementation of a resident oversight and credentialing policy
BACKGROUND This study assesses the impact that a resident oversight and credentialing policy for central venous catheter (CVC) placement had on institution-wide central line associated bloodstream infections (CLABSI). We therefore investigated the rate of CLABSI per 1,000 line days during the 12 months before and after implementation of the policy. METHODS This is a retrospective analysis of ...
متن کاملUsing Serious Gaming to Improve the Safety of Central Venous Catheter Placement: A Post-Mortem Analysis
Central venous catheter related complications are a large burden on our healthcare system. Many of these complications can be prevented by the use of proper technique. One methodology to aid in the teaching nontechnical aspects of procedures, serious games, has not been utilized in our field. The authors therefore set out to design a serious game aimed at teaching proper central venous catheter...
متن کاملA New Algorithm for Detecting Text Line in Handwritten Documents
Curvilinear text line detection and segmentation in handwritten documents is a significant challenge for handwriting recognition. Given no prior knowledge of script, we model text line detection as an image segmentation problem by enhancing text line structure using a Gaussian window, and adopting the level set method to evolve text line boundaries. Experiments show that the proposed method ach...
متن کاملIntensivist supervision of resident-placed central venous catheters decreases the incidence of catheter-related blood stream infections
Catheter-related blood stream infections (CRBSI) cause significant morbidity and mortality. A retrospective study of a performance improvement project in our teaching hospital's surgical intensive care unit (SICU) showed that intensivist supervision was important in reinforcing maximal sterile barriers (MSB) use during the placement of a central venous catheter (CVC) in the prevention of CRBSI....
